Library Forms
Library Membership form
The Undergraduate and Postgraduate students, Integrated students, Research scholars’ Academic and Non-academic staff of Amrita Vishwa Vidyapeetham, Amaravati Campus can be eligible for library membership. Library membership forms are issued at the library or downloaded from the intranet website. All persons eligible to use the library must first register by filling up the prescribed registration form. Application of the students, faculty, and non-teaching staff are to be recommended by the competent authority.
Book Recommendation Form
Faculty members can recommend the books by filling Print/Online Book Recommendation Form with the details about book including title, author, publisher, ISBN, price, etc... The books requisitioned by the faculty members for purchase should be routed through the respective HOD and Principal always with approval.
The active students including research scholars, undergraduate, and graduate students of Amrita Vishwa Vidyapeetham, Amaravati Campus can also recommend the books by Print Book Recommendation Form with details about book including Title, Author, Publisher, ISBN, Price etc. The book(s) requisitioned by the students must be approved by the concerned faculty (usually supervisor) and forwarded through the HOD.
Journal Recommendation Form
Faculty members can recommend the print journals by filling Print/Online Journal Recommendation Form with details about journal including title, publisher, frequency, ISSN, and price. The print journal requisitioned by the faculty members for purchase should be routed through the respective Chairperson and Dean always with approval.

Article / Book Chapter Request form
The Central Library provides a service where the full text of papers subscribed to or available in the Central library can be requested online. This is a free service for educational purposes only, with copyright limitations. Please submit your requests using the online form given below. We aim to respond within one working day. If the requested article is not available in the library, we will make every attempt to obtain the article from other libraries or sources.

Quillbot(Grammar) Registration Form
The faculty members and Research Scholars can register for the Quillbot access facility.
Plagiarism Check form
The Postgraduate students and Research scholar should fill all the column of the plagiarism form and sign in the specified column. Research Supervisor and Heads of the Departments also should sign in the plagiarism form before submission of the article / e-thesis. The scholar should send the soft copy of the article and thesis to a_muthuraj@av.amrita.edu
Book Donation form
The library welcomes inquiries about donations of library books but cannot accept everything. If you wish to donate books to the Central Library, please use our print form.
